Follow these steps to request a road accident report:
- Submit your request by mail for processing. No documents will be issued in person.
- Your written request must include:
- The event number provided when the police report was filed.
- A photocopy (front and back) of your driver’s license.
- Your signature.
- Include a $19 check payable to Ville de Laval.
- Send everything by mail to the following address:
